Community Partners, the Living at Home/Block Nurse Program of Two Harbors, exists to help older persons to live independently in their own homes. We organize neighborhood volunteers and provide support services, information and service coordination.
Community Partners was initiated and developed by area residents. Our vision is to strengthen our community’s commitment to older residents and to enhance the quality of life for older residents and their caregivers.
Community Partners is graciously funded by a variety of local, state and federal sources including the State of MN, Lake County, Arrowhead Area Agency on Aging Older American’s Act Grant Programs, the United Way, the CLIFFS Foundation, Medica Foundation, and many generous local service clubs, individuals and businesses. Donations, Memorials and Honorariums are greatly appreciated.
